SMS Consent Terms
Effective date: June 12, 2026
These SMS Consent Terms describe the text-messaging program operated by BlueMint IQ, Inc. (“CareIQ”) on behalf of participating healthcare providers. They supplement our Terms of Service and Privacy Policy.
1. Program Description
With your consent, CareIQ sends care-related text (SMS) messages on behalf of your healthcare provider. Messages may include:
- Enrollment and onboarding links for your care programs;
- Appointment, check-in, device-reading, and medication reminders;
- Messages and follow-ups from your care team;
- Educational content and program updates related to your care;
- Account notifications such as secure sign-in links.
2. Opting In
You consent to receive SMS messages by providing your mobile number and agreeing to receive texts — for example, during program enrollment with your provider, in the CareIQ patient application, or on a form that references these SMS Consent Terms. Consent to receive text messages is not a condition of receiving care, treatment, or any CareIQ service.
3. Message Frequency
Message frequency varies based on your care programs, your care team's outreach, and your activity — for example, you may receive several messages per week while actively enrolled in a monitoring program.
4. Message and Data Rates
Message and data rates may apply according to your mobile carrier's plan. CareIQ does not charge for SMS messages, but your carrier's standard charges apply.
5. Opting Out
Reply STOP to any message to opt out of SMS at any time. After you reply STOP, we will send one final message confirming your opt-out, and you will receive no further texts unless you re-subscribe. You can also manage communication preferences in the patient application or by contacting your provider. Opting out of SMS does not affect your enrollment in care programs — your care team will reach you through other channels.

7. Privacy of Your Mobile Information
Mobile numbers and opt-in status are used solely to deliver the messages described above. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be sold or shared with any third parties. Health information in messages is handled in accordance with HIPAA and our Privacy Policy.
8. Supported Carriers and Liability
The program is available on major U.S. carriers. Carriers are not liable for delayed or undelivered messages. Message delivery is subject to network availability and your device's capabilities.
9. Changes
We may update these SMS Consent Terms from time to time. Material changes will be posted here with an updated effective date.
